Student-Athlete Paperwork

Each student-athlete will need to complete their paperwork in its entirety prior to participating in any practice or pre-season activities.

The student/athlete paperwork must be completed online.

For returning student-athletes, the Insurance Form, along with a copy of a new physical, and a copy of the front and back of the student-athlete's insurance card need to be uploaded to an existing SportsWare account.

For reference purposes, the entire student-athlete paperwork packet may be viewed here, but it must be completed and submitted online.

In addition to the online paperwork, each student/athlete must have a physical examination conducted before each school year.
